If you need to request records, you can contact one of our Access workers at 715-284-4301 ext. 360, or fill out the form below. 

Cost of Records:

There is a $.25 cents per page cost associated with any records request. An invoice will be provided. Payment is due at time of pick up or has to be paid before they are mailed out.
