The first form is a log sheet to track community service hours. Please follow the directions on the form and ensure there is a log for each location hours are completed. When hours have been completed return signed forms to your social worker.
The second form contains some suggested sites that have been used in the past for completing community service hours.
This is the required paperwork to complete to begin the uncontrollability referral process. Please read all directions thoroughly and carefully. Return completed and signed form to Jackson County DHHS.

DownloadJackson County utilizes SCRAM Systems for GPS monitoring. GPS Monitoring can be used when juveniles are non compliant with conditions, or having escalated behaviors. Below is the GPS Monitor Guidelines.
Reminder: If you tamper with, or attempt to take off your GPS monitor it will send an alert to your social worker and the Children and Families Supervisor immedietly. If you are violating curfew or your assigned schedule then it will send an alert immedietly. If the battery is critically low it will send an alert. Monitoring social worker can check your location and battery charge at any time.
Care of GPS Monitors: Showering should not be harmful to a GPS monitor but bathing and swimming is not advised. Remember to keep your GPS Monitor charged, it's recommended to charge while you sleep if possible.
